i have just sent the above email off to ATOS at customer-relations@atoshealthcare.com

to state clearly what the problem is...it is now 14/02/2013 and nobody at ATOS has signed to say that they have received my questionnaire - at the address stated upon their envelope...this is worrying because the return date is in a week: 21/02/2013
